PATIENT TERMINATION OF RELATIONSHIP POLICY

This policy describes how the patient-provider relationship at Lucidity Health may end, whether you choose to stop care or Lucidity Health initiates the change.

Patient-Initiated Termination

You may end your care with Lucidity Health at any time, for any reason, without notice. When you elect to discontinue treatment, we will document the discontinuation in your record, explain any material health considerations associated with stopping treatment, and offer relevant follow-up options, such as lab testing at standard rates. We are not obligated to provide free ongoing monitoring once you have elected to discontinue care, and we will provide access to your records upon request.

Clinic-Initiated Termination

Lucidity Health may end the patient-provider relationship for the reasons described below.

Non-Payment. For an account in delinquency, we will attempt to contact you up to three times over a 10-day period by text, email, or other reasonable means to resolve the balance. If the delinquency remains unresolved at the end of that period, we will proceed with a 30-day termination notice, sent the same day through certified mail with electronic return receipt, first-class mail, and email or patient portal message.

Non-Compliance with Treatment. This includes failure to complete required monitoring labs after two documented requests, refusal of clinically required monitoring or follow-up, obtaining the same or a duplicate controlled substance from another prescriber without disclosing it to us, self-adjusting a prescribed dose against provider direction after being counseled, or repeatedly missing scheduled appointments without a valid reason.

Fit and Continuity of Care. In some cases, we may determine that we are not able to establish the collaborative provider relationship our care model depends on, or that we have an ongoing clinical disagreement about the appropriate treatment approach that cannot be resolved. As described in our Scope of Care policy, we are selective about fit because it leads to better outcomes for our patients, and this same principle applies if that fit breaks down after treatment has begun.

Change in Location. Our providers are licensed to provide care only in the states where they hold active licensure, currently Oregon and Washington. If you relocate outside of these states, we may not be able to continue your care by telehealth, and this may result in termination of the relationship for that reason alone, unrelated to your compliance or fit with our practice.

Same-Day Termination. The following circumstances may justify ending the relationship immediately, without the 30-day notice period described above. We will still provide you with a copy of or access to your records upon request.

  • Physical violence or a credible threat of violence against staff, providers, or other patients

  • Verbal abuse, harassment, or intimidation that creates a safety concern for staff or other patients

  • Sexual harassment or inappropriate sexual conduct directed at staff

  • Possession of a weapon on clinic premises

  • Theft, attempted theft, or diversion of medication or clinic property

  • Falsification or alteration of a prescription

  • Confirmed criminal activity on clinic premises

  • Being under the influence of drugs or alcohol in a manner that threatens the safety of staff or other patients

During the Transition Period

If your care is ending under a 30-day notice, you will continue to have access to urgent care needs related to your treatment through the effective date, along with any applicable transition option, such as a limited bridge supply, a referral to another provider, or a documented tapering plan, for 30 days from the date the notice is sent or until you establish care with a new provider, whichever comes first. We encourage you to begin establishing care with a new provider as soon as possible, and a list of resources to help you locate one is available upon request.

Your Records

Access to your medical records is never withheld, delayed, or conditioned on payment. You may request a copy of your records, or ask that they be transferred to a new provider, at no charge, and requests are processed within thirty days.
