Washington Consumer Health Data Privacy Policy

This policy applies solely to residents of Washington State and describes how Lucidity Health LLC ("Lucidity Health," "we," "us") collects, uses, and shares Consumer Health Data as defined under the Washington My Health My Data Act (RCW 19.373). This policy is separate from our general Privacy Policy and our HIPAA Notice of Privacy Practices, which govern other categories of information.

1. What Is Consumer Health Data

Consumer Health Data means personal information that is linked or reasonably linkable to a consumer and that identifies the consumer's past, present, or future physical or mental health status. This includes information such as health conditions, treatment, diagnoses, medications, bodily functions, reproductive or sexual health information, and biometric or precise location data that could indicate an attempt to seek health care services.

2. Categories of Consumer Health Data We Collect

We may collect the following categories of Consumer Health Data:

• Information you provide directly, such as intake forms, health history, symptoms, and treatment goals

• Information generated through your care, including lab results, diagnoses, medications, and treatment protocols

• Appointment and scheduling information that indicates you are seeking or receiving health care services

• Website interaction information that could indicate an attempt to seek health care services

3. Sources of Consumer Health Data

We collect Consumer Health Data directly from you, including through intake forms, consultations, and ongoing treatment. We may also receive Consumer Health Data from outside parties who assist in providing your care, such as laboratory and prescription fulfillment partners.

4. Purposes for Collecting Consumer Health Data

We collect and use Consumer Health Data to:

• Provide, coordinate, and manage your medical treatment

• Process payment for services

• Communicate with you about your care, including appointment reminders

• Maintain required medical records and comply with legal and regulatory obligations

• Operate and improve our patient membership programs

5. How We Share Consumer Health Data

We do not sell Consumer Health Data. We may share Consumer Health Data with:

• Outside parties who assist in providing your care, such as laboratory and prescription fulfillment partners

• Service providers who process data on our behalf under contract

• Affiliated Lucidity Health operating entities, where care is coordinated across locations

• Government or regulatory bodies when required by law

We do not share Consumer Health Data with third parties for advertising or marketing purposes without your separate, affirmative consent.

6. Your Rights

If you are a Washington resident, you have the right to:

• Confirm whether we are collecting, sharing, or selling your Consumer Health Data

• Access your Consumer Health Data

• Request deletion of your Consumer Health Data

• Withdraw consent for the collection or sharing of your Consumer Health Data

8. Changes to This Policy

We will update this policy and the effective date above if our practices change. Material changes involving new categories of data or new sharing purposes will not take effect until we obtain your affirmative consent.
