Total testosterone below 300 ng/dL can be considered normal by insurance companies. So, low testosterone is not seen as a medical issue.

Most men will benefit from testosterone levels of 800 ng/dL to 1200+ ng/dL.

Low testosterone is best treated using hormone replacement therapy (HRT). This is best done with intramuscular (IM) or subcutaneous (SQ) injections.

We promote testosterone injections using the SQ route using insulin-sized syringes. We also have creams and even oral testosterone (which is new on the market).

Ideally, testosterone is injected twice weekly to avoid TRT lows that can occur. Once weekly or especially less will only result in mild, short-lived benefits.

Other products exist, including intranasal testosterone, topicals, patches, or pellets, but none compare to direct administration.

Why We Don't Use Oral Testosterone

Oral testosterone has gained popularity, largely driven by marketing around convenience. However, our clinical experience and commitment to optimal patient outcomes lead us to a clear conclusion: oral testosterone underperforms and doesn't justify its use in a hormone optimization practice.

The Clinical Reality

Achieving therapeutic testosterone levels requires very high doses. Even with aggressive dosing protocols, many patients struggle to reach a total testosterone of 1,000 ng/dl. This is simply not acceptable when injectable protocols consistently deliver stable, physiological levels with significantly lower doses and superior side effect profiles.

Cost and Convenience vs. Efficacy

The convenience factor of oral administration sounds appealing in theory. In practice, patients end up taking multiple capsules daily without achieving the results they need. The cost burden, combined with suboptimal outcomes, makes oral testosterone a poor investment for patients pursuing genuine hormone optimization.

Our Approach

Oral testosterone should not be confused with oral anabolics, which serve different therapeutic purposes and are used on different clinical principles. We've chosen to move away from oral testosterone entirely in favor of systemic protocols using injectable therapies, which allow us to achieve consistent, stable hormone levels, use lower total doses for better safety profiles, deliver predictable patient outcomes, and avoid the variable absorption issues that plague oral formulations.

Our belief is simple: the best hormone replacement therapy should be based on what works best for your body, not what's easiest to market. Injectable protocols accomplish that.

The gold standard is systemic testosterone injections that are easy to self-administer using insulin-sized syringes. Testosterone creams are available. Please read our statement about oral testosterone and why we avoid its use.

However, we have some base labs we like to start with and we monitor them every 2-6 months dependent upon your stage of therapy. It takes 2-6 months to become “optimized” or “dialed-in”.

TRT temporarily suppresses your body’s natural testosterone production, but this effect is reversible. When therapy is discontinued, we use medications such as HCG to stimulate the testes and enclomiphene to boost pituitary signaling for LH and FSH. In some cases, FSH is added to accelerate recovery, helping natural testosterone production return within 1 to 6 weeks.

If you are on HCG or Enclomiphene, you will continue making our own natural testosterone. We then use testosterone to supplement or boost your natural levels until you feel benefit.

Human chorionic gonadotropin (hCG) is an injectable that helps balance hormones. HCG effectively balances testosterone, enhances its effects, and keeps you in a stable mood. Most notably, it prevents testicular atrophy (shrinking testicles, which occurs only in some males) and maintains fertility in men who wish to have children while on TRT.

Enclomiphene is an oral SERM, a selective Estrogen Receptor Modulator. It essentially tells the brain to keep telling the testicles to keep doing their job (making testosterone and sperm) while on TRT.
